Model release agreement

Consent. The performer confirms they are at least 18 years old and voluntarily participated in the recorded content. They consent to its capture, storage, publication, distribution, display, sale, and sublicensing by RevealThis and the identified creator.
Rights granted. The performer grants RevealThis and the creator a perpetual, worldwide, transferable right to use the recorded content and the performer's likeness, name, voice, and biographical details in connection with that content.
Compliance. The performer confirms participation was consensual and free from coercion, intoxication, fraud, or incapacity, and agrees to provide government-issued identification for secure age and record-keeping checks.
Record keeping. The signed release is stored with the verified performer record and must remain linked to covered published content for compliance review.
Contract wording is fixed. The generated PDF adds the verified factual details, signature evidence, signing time, IP address, and release identifier.
